Bring your driver's license and eyeglasses if you wear them while driving
To be efficient, drivers with any of the following conditions can be better prepared for their exam by bringing in the proper forms and documentation on the day of their physical exam.
DIABETICS (WHO TAKE INSULIN)
Need to have their doctor complete MCSA-5870 form to be certified
DOWNLOAD MCSA-5870 FORM
These drivers must have their Treating Clinician (who prescribes them their insulin) complete the ITDM Assessment Form MCSA-5870, attesting that the individual has a stable insulin regimen and properly controlled diabetes. These drivers are required to provide this form to the Dr. Decker within 45 days of completion by the Treating Clinician.
DRIVERS THAT USE A C-PAP
Drivers that use a C-PAP device for sleep apnea are required to demonstrate 70% or greater compliance of their device over the last 3 months by bringing a copy of their compliance report
