Important Info about Medical Cards for Non-CDL Drivers (NJ)
It is important that employers understand that their workers may need a DOT Physical Examination performed if driving a company vehicle and/or pulling a trailer if the total weight is over 10,001 lbs. Expensive penalties can be incurred. Read below:
- NJ commercial drivers operating a vehicle with GVWR or gross combination weight (weight of included trailer and equipment) of 10,001 pounds or more N.J. Admin. Code § 13:60-1.2 and 49 CFR 390.5 enforced under the provisions of 39:5B-29.
- Violation N.J. Stat. § 39:5B-29 Any person who violates the provisions of this act or any rule or regulation adopted pursuant thereto shall be subject to a penalty of not less than $100 nor more than $5,000.00 for the first offense, nor less than $200 nor more than $10,000.00 for the second offense, nor less than $500 nor more than $25,000.00 for the third or any subsequent offense.

- On-site physical examinations can be performed at your shop or headquarters. Early morning or late Hours. He will schedule with you.
- Off-hours access. Dr. Decker can perform physical examinations for multiple drivers outside of his Chiropractic treatment hours.
- Company Discount. We can work with you and provide a discount if we are performing exams on several or more drivers
- Get all of your employees to be on the same examination date. We can work with you to get your exams around the same time period. Make it easier for you and get your qualified drivers to all fall on the same expiration date. This way it's easier to manage their qualification and make sure they are not driving illegally.

- Determine if the visit will be on-site, during office hours, or outside of office hours for Dr. Decker
- Schedule a date and time with Dr. Decker. He will contact you on his personal cell and setup the visit.
- Download a copy for each driver of the MEDICAL EXAM REPORT FORM MCSA-5875
- The first page must be filled out correctly and clearly.

Pre-Screening Drivers with Medical Conditions
To be efficient, drivers with any of the following conditions can be better prepared for their exam by bringing in the proper forms and documentation on the day of their physical exam.
DIABETICS (WHO TAKE INSULIN)
Need to have their doctor complete MCSA-5870 form to be certified
DOWNLOAD MCSA-5870 FORM
These drivers must have their Treating Clinician (who prescribes them their insulin) complete the ITDM Assessment Form MCSA-5870, attesting that the individual has a stable insulin regimen and properly controlled diabetes. These drivers are required to provide this form to the Dr. Decker within 45 days of completion by the Treating Clinician.
DRIVERS THAT USE A C-PAP
Drivers that use a C-PAP device for sleep apnea are required to demonstrate 70% or greater compliance of their device over the last 3 months by bringing a copy of their compliance report

Exam Day
- Drivers bring DRIVERS LICENSE and EYEGLASSES if they wear them for driving
- Drivers should avoid urinating immediately before exam. They will NOT be drug tested. However, drivers will perform a urine dip stick test to look for glucose, protein, or blood in the urine. A bathroom must be accessible for the exams.
