Bring With You
✅ A valid driver’s license
✅ A list of current medications (including dosages and prescribing doctor’s name)
✅ Corrective lenses or hearing aids (if you use them)
✅ Any necessary medical clearance letters (e.g., for past surgeries, heart conditions, sleep apnea, or other medical issues)
Diabetics who takes insulin
Need to have their doctor complete MCSA-5870 form to be certified

These drivers must have their Treating Clinician (who prescribes them their insulin) complete the ITDM Assessment Form MCSA-5870, attesting that the individual has a stable insulin regimen and properly controlled diabetes. These drivers are required to provide this form to the Dr. Decker within 45 days of completion by the Treating Clinician.
Drivers who use a CPAP
Drivers that use a C-PAP device for sleep apnea are required to demonstrate 70% or greater compliance of their device over the last 3 months by bringing a copy of their compliance report

Keys To Success
- If you have high blood pressure, take your prescribed medication as directed.
- Avoid caffeine, nicotine, and salty foods 24 hours before the exam to prevent a temporary spike in blood pressure.
- Stay Hydrated for the Urine TestThe DOT physical includes a urinalysis, which checks for blood sugar levels (diabetes), kidney function, and other health markers. Drink plenty of water before your appointment to ensure you can provide a sample.
- Get a good night's sleep. Try to get at least 7-8 hours of sleep the night before.
- If you have any chronic conditions (e.g., diabetes, sleep apnea, heart issues), bring relevant documentation from your doctor. If you use a CPAP machine for sleep apnea, bring your compliance records. Text us if you have any questions. We can send you any clearance letters you may need before your visit.
