Date Of Birth: Click on the YEAR in the top left, it will allow you to scroll up to your birth year. Then, select month and date
Existing Patients: For instant booking, your Name, DOB, cell number, & e-mail address must match exactly to our system. Otherwise, we must approve it.
New Patients: If we have the wrong date of birth and we don't catch it at the office during your visit, your insurance claims will be rejected.
